Healthy Communities promotes healthcare and support services through outreach, awareness, linkage to care and support. Healthy Communities serves to identify the unique and integral needs of our communities and neighborhoods and to establish strategic plans to address them. Service delivery will make healthcare and support services accessible through flexible delivery of services. AmeriCorps members will conduct community outreach, education, patient engagement, and link patients to Community Health Center, Inc. care and referrals to community-based assistance.
These AmeriCorps Members serve as critical figures in providing healthcare education and outreach services within Connecticut communities.
AmeriCorps members will participate in statewide and local activities which promote awareness of health and health services, as well as various activities related to outreach, education, linkage to care, retention in care, Covid vaccination and engagement in healthcare and support services. At the end of the first program year, the AmeriCorps members will be responsible for supporting increased health knowledge and improved access to care among individuals in the communities served. In addition, the AmeriCorps members will leverage 10 volunteers who will be engaged in the support of community events and outreach activities.
