Healthy Communities is the Community Health Center Inc.’s Public Health AmeriCorps program designed to create cohesion in vulnerable communities and to promote healthcare and support services through outreach, awareness, linkage to care and support. Healthy Communities serves to identify the unique and integral needs of our communities and neighborhoods and to establish strategic plans to address them.
Service delivery will directly address the public health crisis in vulnerable communities and make healthcare and support services accessible through flexible delivery of services. AmeriCorps members will conduct community outreach, education, patient engagement, and link patients to CHCI care and referrals to community based assistance. Additionally, AmeriCorps members will participate in statewide and local activities which promote awareness of health and health services.
Grant Deliverables: Healthy Communities will have 15 AmeriCorps members who will participate in various activities related to outreach, education, linkage to care, retention in care, Covid vaccination and engagement in healthcare and support services in Enfield, Bristol, Norwalk, Stamford, New Britain, New London, Meriden, Middletown, Danbury, Waterbury, Clinton, Old Saybrook, Hartford, Groton and a variety of community based sites. At the end of the first program year, the AmeriCorps members will be responsible for supporting increased health knowledge and improved access to care among individuals in the communities served. In addition, the AmeriCorps members will leverage 10 volunteers who will be engaged in the support of community events and outreach activities. After their year of service, 7 AmeriCorps members will pursue future engagement in the public sector.
